A quick response but: An aortic dissection is very different from a heart attack, and also, someone's appearance (by which people generally mean body shape/size) is an immensely poor indicator for heart health.

My friend's mom, who was in her 50s at the time and very fit for her age (went running basically every day, among other things), had an aortic dissection a few years back. Luckily, most people survive and she was able to get treatment, but yeah, just another scary random thing that can happen. :/ I'm glad it was at least quick and painless for him.

^ 2nd that. One of my music teachers who played trumpet was an avid runner, did a ton of marathons, super fit and skinny. One morning was running a marathon - collapsed and died out of nowhere. Was in his 50's too.
